As mentioned previously, my friend Jeni is very good with knowing things about fertility. Not only about herself, but just seems to be well knowledged on this subject and has agreed to do "ask jeni" once a month.
She is NOT a fertility expert nor should you take her own opinion/advice she gives over that of your doctors. She is doing this to provide insight, hope and to perhaps give you another direction to look into when trying to conceive.
In the comments section, the first three people who post questions for Jeni will be answered. I am not sure if Jeni will have time to answer any otheres, but will do her best, and will answer at least the first three. So...I am not sure what has gotten me to do this, but something is just telling me to go for it...so I am going for it...
My husband and I are talking seriously about trying to concieve #1 and I am a bit anxious about it (just a big change I guess), but anyways...just wondering if you a little one is in the near future or if we are going to have difficulties trying to concieve???
Hi Jeni , We have been ttc #1 for 2 1/2 years now without fertility treatment. We are thinking it may be time now to get more aggressive with regards to the whole process. I have PCOS and we are thinking of giving clomid a try. I have regular cycles but just dont ovulate. I am on a 3cycle BCP until march to shrink any cysts that may be present before trying something else. Do you think we are on the right route, is there any advice or intuition you may have for us?

For someone who has no known fertility problems, at what point would you seek out a little help (from like clomid, etc?) We are almost to the two year mark.

I am not able to say whether or not you will be conceiving right away, but I do think Cheri can offer a little insight on that question with a TTC prediction. : ) You just do not know until you give it a try how long it will take and if you will have trouble conceiving. I always thought I would just turn up pregnant the first time without protection, but it took 18 months. Other people nail it on the first try. Please refer to Cheri's Blog called "Unsure about Ovulation" posted January 12. I wrote a lot of info on tracking and understanding ovulation, and even gave some good resources and preferences based on my own experience. Cheri posted it word for word. Please let her know if you need further help and have her email me with your email address. Doctors usually will not consider fertility treatment until you have tried for 12 months with no success. PCOS makes TTC very hard, but NOT impossible. I do think that you are on track by starting BCPs to shrink the cysts and considering Clomid. If you were to just go for it with existing cysts, your chances of conception can be lowered because a cyst that is large in size can inhibit ovulation from other follicles. Stick with the BCPs for 3 months and DEFINITELY go for the Clomid. Clomid has worked wonders for women with PCOS, but does have some unpleasant side effects. The side effects are temporary and well worth it when you finally get your bundle! You can also google "Clomid success stories" and see what other women have had happen for them! This is what DH and I did to get pregnant.
You may also want to ask your Dr for some blood work to check Progesterone, thyroid, PROLACTIN, and other hormones that can affect TTC. They are referred to as the "Cycle day 3" and "Cycle day 21/ 7 days past ovulation test." If you have an extended cycle and track ovulation, the "cycle day 21" test will actually need to be done at 7 days past ovulation. Usually with fertility treatment, you will take Prometrium (progesterone) after ovulation (3DPO-AF) to support a pregnancy, if it did indeed work. Some doctors will also have you take Estrace 2mg(estrogen) on Cycle day 10-16 to help biuld mucus and endometrium. A lot of doctors will not do the estrogen though. Taking Clomid will require a lot of work.. You will have to CHART your cycles regularly and use OPKs. OPKs are GREAT for Predicting ovulation, whereas charting only confirms it 3 days after it has occurred. Fertility monitors on Clomid or with extended cycles are not recommended because they do not work for women with cycles longer than 40 days.
Regular ovulation tests (OPKs) are the best. Also posted on Cheri's blog is more info on charting. Refer to January 12 posting titled "Unsure about ovulation" for a lot more info. : ) GOOD LUCK!!!!!

You are WAY past the mark where fertility treatment can be considered. ONE YEAR having unprotected sex regularly with no resulting pregnancy is when they will start to look more into the situation. PLEASE go see your OB/GYN and discuss this issue. At the appointment, let your Dr know what your cycles are like (how long, heavy, painful, regular or irregular, etc) and see where he/she wants to go with the issue. The first treatment is usually Clomid. Once you have the appointment and a "game plan" refer back to Cheri to get back in contact with me. You will aslo need to start tracking your ovulation cycles. For info on that, refer to Cheri's January 12th posting called "Unsure about Ovulation." Good Luck!!!!!
